You can submit a claim for bodily injury and/or property damage by completing the
online form
and mailing it to:
Office of the General Counsel, Department of Transportation,
605 Suwannee Street, MS 58, Tallahassee, FL 32399-0458.
You can also e-mail the form to
dotclaims.review@dot.state.fl.us.
